Patient Information

Please bring a photo ID, your insurance card, and your physical therapy order or referral if one has been provided. If you have relevant surgical or imaging information available, you may bring that as well.

Wear comfortable clothing that allows you to move easily and gives your therapist access to the area being treated when appropriate.

Gowns may be provided if needed. 

Your first visit will typically include a review of your history, an evaluation of your movement and function, discussion of your goals, development of an individualized plan of care, and you will be provided with a schedule for any follow up visits. 

Referral requirements can vary depending on your insurance plan and individual situation. Contact our office if you are unsure whether a physician order or referral is required.

Coverage and benefits vary by insurance plan. Our office can assist with benefit verification, but patients are encouraged to confirm their individual physical therapy benefits with their insurance carrier.

We ask that you provide at least 24 hours’ notice whenever possible if you need to cancel or reschedule an appointment. We understand that emergencies and unexpected situations can occur, and we will always take those circumstances into consideration.

Late cancellations or missed appointments without adequate notice may be subject to a $25 fee.
